Our office recognizes that there are many reasons why dental care is postponed or not completed. Whether it is a regular dental cleaning or dental treatment, receiving dental care is important for your general health. In an effort to provide the necessary care that our patients have overlooked or postponed, perhaps due to financial costs, Dr. Raia and his staff have developed an in office dental program. This program is designed to assist those patients without insurance. The following is information about our 3 dental plans. Silver Plan
$350 per year | Includes: 2 Prophy Cleanings | Code: D1110 | Exams as needed | Xrays as needed |10% Off all other services| Additional fee will be charged for diagnosed periodontal gum disease treatment. Each patient will be advised of their periodontal case type.
Gold Plan
$525 per year | Includes: 3 Prophy Cleanings | Code: D1110 | Exams as needed | Xrays as needed | 15% Off all other services| Additional fee will be charged for diagnosed periodontal gum disease treatment. Each patient will be advised of their periodontal case type.
Platinum Plan
$700 per year | Includes: 4 Prophy Cleanings | Code: D1110 | Exams as needed | Xrays as needed | 20% Off all other services| Additional fee will be charged for diagnosed periodontal gum disease treatment. Each patient will be advised of their periodontal case type.

Plan Rules, Limitations, and Exclusions
Premium is paid in full at time of first appointment is made. All other appointments must be prepaid upon being scheduled. Premium and prepayments are non-refundable and non-transferable between dates. No rescheduling, no cancelations, no exceptions.
A plan upgrade may be changed within 30 days of plan selection by paying the difference in premiums. However, the anniversary date will not change. This plan cannot be combined with any other type of insurance, workman’s comp, or disability. CareCredit plans are offered for terms of 24 months or greater for all 3 plans. Additional family members, after first enrollee, will receive 10% off the lowest purchased plan. Referrals to specialists and the cost associated with that treatment are not covered by this plan. Dental treatment deemed to be outside the scope of the doctor’s experience will be referred to the appropriate specialist in that field of dentistry. Policy plans are subject to change without notice.
Within 10 days after initial office visit, patients may return to the office and participate in any of these plans. Fees from the initial visit may be applied to the dental plan selected. Policy plans are subject to change without notice.
